The City of Montgomery Wellness Program is available to all employees insured on the City's Group Health Plan. The program is administered according to federal rules permitting employer-sponsored wellness programs to seek to improve employee health and/or prevent disease. This program is only available to City employees. Retirees are not obligated to participate.
Notice Regarding Wellness Program
The Wellness Program is comprised of the Health Risk Assessment (HRA) and Tobacco Attestation.
Health Risk Assessment (HRA)
Beginning January 2022, the annual wellness screening will have a name change from Annual Health Assessment (AHA) to Health Risk Assessment (HRA).
All employees insured on the City's Group Health Plan are encouraged to complete the HRA anytime between January 1st - September 30th each year. Employees have the option of completing the HRA at the City Wellness Centers for free or with your own private physician.

Employees who do not complete the HRA between January 1 - September 30 will be charged $25.00 per pay period through payroll deduction. This deduction will start January of the following year and will last for a full year.
Employees who complete the HRA at Premise Health are able to view their incentive and compliance online.
Tobacco Attestation
All employees insured on the City's Group Health Plan must either attest to being a tobacco user or not being a tobacco user. Tobacco users will pay an additional $12.50 each pay period through payroll deduction for the health insurance.
