Overview of Employee Benefits

Health Benefits

BCBSBlue Cross Blue Shield is the provider for health and dental coverage for City employees and retirees.  It is intended to help you and your covered dependents pay for the costs of medical care.  The plan does not pay for all of your medical care.  You may also be required to pay deductibles, co-payments and coinsurance.

 

Active Employees
Health Management Plan (HMP) is a managed healthcare plan. It offers reliable coverage.  Under this plan, you are required to pay a deductible and co-insurance which is a percentage of the cost a doctor or hospital charges.  Once the calendar year deductible (½ by the City and ½ by the employee) is met the insurance pays 80% and the employees pay 20% with two exceptions: certain preventive benefits are covered by the Affordable Health Care Act at 100% with no deductible and hospital per diem charge is covered at a 100% once calendar year deductible is met.

Health Management Plan (HMP) - for active employees.  

Preferred Provider Organization (PPO) the plan allows you to visit whatever in-network physician or healthcare provider.  A copayment is required based on the group plan and it is the amount you pay before your insurance begins picking up any of the cost.

Retirees Under 65 -
Preferred Provider Organization (PP0)—same as active PPO plan.
Health Management Plan (HMP) –same as active HMP plan.

Retirees Post 65 - 
Preferred Provider Organization (PP0)—same as active PPO plan.  Plan pays secondary to Medicare.

BCBS Dental—The plan is intended to help you and your covered dependent pay for the costs of dental care under the group plan.  The plan covers Diagnostic and Preventive care (exams and cleanings), Restorative (fillings and root canals), Supplemental (oral surgery and anesthesia), Periodontics (gum disease).  The benefits are subject to the terms, limitations and conditions of the group contract.

Prescription Prescriptions drugs:  The City offers prescription drug plan for covered employees and their dependents through Employer Health Options (EHO).   The plan recognizes any Willing Provider.  Any Pharmacy that agrees to meet the plan’s conditions and electronically submits your prescription can fill it for you.  The calendar year deductible is $250 (maximum of 3 per family) which starts in January of each year.

Other Employee Benefits

Alliance Insurance Group (AIG) manages the Flexible Spending Plan.  A flexible spending account is an employer-sponsored benefit that allows you to pay for eligible expenses on pre-tax basis.  They are:
Medical FSA Account-employees contributes to plan through payroll deduction with pre-tax dollars for medical expenses, dental expense, and vision care.
Dependent Care Account-employees contributes to plan through payroll deduction with pre-tax dollars     for reimbursement for daycare expenses for children, after school programs or dependent parents.

Vision Service Plan (VSP) - A provide you with an affordable eye care, eyewear and choices of providers.

Employee Assistance Program (EAP)This program is managed by American Behavioral Benefits and is offered to all employees and eligible dependents. All visits must be pre-approved.  EAP provides confidential Assistance with: Family & Marital issues, Legal & financial Referrals, Stress Related Issues, emotional issues and Alcohol &Drug Abuse. Substance Abuse

Substance abuse Bradford Health Services—This the program for Substance Abuse and Rehabilitation counseling.

Standard As a City employee, you are provided a basic life insurance policy at no additional cost.  This insurance is currently written by The Standard.  The amount of the insurance is 1 times annual salary, rounded to the next multiple of a $1,000s, and capped at $50,000.

AD& D coverage provides an additional 1 times your annual salary, rounded to the multiple of a $1,000s and capped at $50,000 active employees only.

DentalIn January 2016, the City awarded Cost Plus Dental a contract to provide care to its employee, retired employees, and their dependents.  One year commitment required and it automatically renewed.

CareHere  Primary Care and Wellness Center (CareHere) 

CareHere provides primary medical care and extensive wellness services for all active employees, retirees and their eligible dependents by appointment.  Requirements:  must registered as a user and must be on the City Health Insurance Plan.  They can dispense 180+ generic medication (free).

YMCA/City Partnership- The City of Montgomery and the YMCA of Greater Montgomery have joined together to offer a great wellness benefit to all City employees and their family members.  The City will pay $20.00 per month toward your monthly bill and the $100 one time sign-up fee is waived.  This can be done during open enrollment or as a new hire.  A new hire must join within 60 days.  You may join the YMCA by contacting the YMCA at (334) 269-4362
